Collars, sweat, razor bumps and hormones

Acne on neck

Acne on neck usually comes from sweat, friction and hair products. Deep, tender bumps on the side often follow hormones, and bumps only where you shave may be razor bumps instead.

What if you could wear any collar without thinking about your neck?

What acne on neck is

Acne on neck skin forms the same way it does on the face: a pore clogs with oil and dead skin cells and bacteria grow in the plug. NIAMS notes that acne often appears beyond the face, and MedlinePlus lists sweating and high humidity as triggers. The neck adds collars, straps and hair right on top.

Causes of acne on neck

Hormones

Women with acne along the lower face, jawline and neck that keeps coming back despite treatment may do well on a birth control pill or spironolactone, says the American Academy of Dermatology. Spironolactone is not prescribed for acne in men. Lower-face breakouts that flare with the cycle point to hormones (PMID 42312792). If the bumps climb from the neck onto the face, the usual suspects change to phones, pillowcases and makeup, all covered under cheek acne.

Friction & Sweat

Pressure from helmets, tight clothes and backpacks makes acne worse (NIAMS). Athletes can break out exactly where gear presses on the neck (AAD). Wear loose, moisture-wicking clothes, change into clean ones after a workout, and pat sweat away instead of rubbing.

Hair Products

Styling oils can leave tiny bumps on the back of the neck, where hair rests (AAD).

Treatments

Over-the-Counter Actives
Salicylic acid works well for acne caused by friction (AAD). A benzoyl peroxide wash also helps, but it bleaches collars and towels (MedlinePlus). Adapalene 0.1% gel is sold without a prescription (FDA).

Small, itchy bumps that all look the same and ignore acne washes may be yeast rather than acne, which is how fungal acne behaves.

Lifestyle Habits
Wash after sweating and don't pop: squeezing pushes pus and bacteria deeper (AAD). A phone wedged between shoulder and cheek touches both spots at once, so check whether the guide to acne on cheeks describes your other side too.
Prescriptions
A dermatologist may combine an antibiotic with benzoyl peroxide for three to four months, or offer hormonal therapy to women (AAD).

Two lookalikes that are not acne

Razor bumps (pseudofolliculitis barbae) form where shaved, curly hairs curve back into the skin. If you stop shaving, they usually thin out within a month and clear within three (AAD).

Acne keloidalis nuchae causes acne-like bumps on the back of the neck and scalp. It is most common in Black men, often starting between ages 14 and 25. It is not acne, it can scar and cause hair loss without treatment, and home remedies with acids can burn, per the AAD. See a dermatologist.

When to see a dermatologist

  • Bumps at the back of the neck that scar or come with hair loss.
  • Deep, painful cysts.
  • Acne on neck skin with no change after six to eight weeks of treatment.

Quick answers

Is it okay to pop neck pimples?

No. Squeezing pushes pus and bacteria deeper, which means more inflammation and a higher chance of a scar.

How can you tell if acne is hormonal or bacterial?

All acne involves bacteria. A hormonal pattern sits on the lower face, jaw and neck and flares around your period.

What is the hardest acne to get rid of?

Deep nodules and cysts. They sit under the skin and need a dermatologist.

What do PCOS pimples look like?

They have no special look. Acne after the teen years that resists usual treatment, together with irregular periods or extra hair growth, is worth a check for PCOS.

What is lymphatic acne?

It is not a medical term that dermatology or health agencies use. A deep lump in the neck that does not behave like a pimple needs a doctor.

Collar line still breaking out?

If the zone fixes haven't settled it, acne.today grades the acne from a photo and a few questions and points to the likely cause.
